I got this message when I tried to fetch an item. However, when I tried again it did work, this error was a one-time thing.
Me too, often when I have been working on a document for a while, and then try to proceed to the next image. Reloading makes it work, but ideally it would just proceed naturally.

An update on this - the blank page with ‘unable to open’ text is gone, but now when I move to the next document (click the one next in the images list - it would be really nice to have a ‘next’ function, as others have said!) after a while, I get the current document (the one I have just been working on) stretched, and I have to reload again.
This happens to me very frequently. What is weird is that when I try to move to the following page, the image remains stuck, superimposed on a message that says that the new item cannot be uploaded (as in Maayan_Aner’s message above), but the transcription correctly proceeds to the item I am trying to upload. Only going back to the list fixes the issue
